Cloth is what I mostly use - you need plastic pants that fit right to go over the diaper - but yes they can still leak (but not often) - keep a washable under pad there just in case - no problem. (don't use a disposable one! - Too light, will end up on the floor where it won't help!)

No need to change at night when I use a premium overnight diaper. (NorthShore Care MegaMax) I get at least 12 hours with no leaks when I'm in bed. I get 6-9 hours of straight sleep and drink all I want or want.

I use a NorthShore MagicSorb Air Disposable UnderPad if there's ever a leak. (I have a $1,400 mattress.) I put it between the mattress and the sheet so it won't end up on the floor or crumpled up and useless. Since it stays dry I only have to change it once a month. (Sheets every week of course.)
