Body worn appliance and Pubic Pressure Urinal

I am surprised that no one seems to use these appliances to get out of adult nappies for a period. I have been using a pubic pressure urinal from the NHS for a year and a half as I have a retracted penis. A belt holds a thick rubber cone over my penis and it is kept close to my body with rubber under straps which fit in my groin. From the come is attached a rubber tube which leads to a standard leg bag. This appliance enables me to go out and shop, walk etc without wearing a nappy as the urine glues straight into the bag. I am unable to wear a sheath but this is a great alternative and once you have the belt and cone the running costs are very modest. Not suitable for wear in bed or long periods sitting but for walking around it is great. In the UK the supplier is Jade Euro Med who have wonderful nurses to fit the appliance in the first instance. Highly recommended if sheath have proven unsuitable and you want a break from nappies.

I use a condom catheter and collection bag at night.

I have heard of something like that called a Postmans friend but not seen them in Health shops here in Canada. I guess they would be available online though. I have never thought of something like that but I guess you can't wear shorts in summer.
 
For anyone who has not seen a body worn or pubic pressure urinal, these are the pictures. The bag is normally strapped to the leg and emptied life any other leg bag. I can recommend this set up for man that wants to take a break from adult nappies.

Never seen that before! If it works for you, that's wonderful! The pictures totally made me think of a medieval torture or kink device, kinda like the funny looking bird-beak like masks.

I also was born with a retracted penis and after several things the diaper works well.I'am also autistic +dd.Thanks

I agree, I have tried a Cunningham clamp and it was absolute agony and took it off after 25 minutes. The pubic pressure urinal can be worn all day with ease and for anyone with a small or retracted penis gives an option away from adult nappies. The NHS supplies mine via a company called Jade Euro Med in the UK. The specialist nurses that fit the device onto you are simply fantastic and the staff at Jade are really helpful. For anyone who has to buy their own supplies, once you have the main appliance then the only cost is a bag a week and a foam pad that fits between the body and the rubber urinal. It may look gruesome but it works and it is great to be out of nappies.

That can be a problem if you don't adjust it correctly. Did your urologist explain how it's supposed to fit, or better yet help you with the initial fit? The idea is to gently compress the area directly under the urethra, not to just clamp it down crushingly tight. If it hurts, you've got it on wrong!

When they're adjusted properly, they're quite comfortable - I've used Cunningham clamps for several hours a day, for weeks at a time, and never had any significant discomfort.

I tried to clamp...HOW THE HELL ANYONE can live like that is beyond me. OUCH! I tried putting it on lightly and I leaked...any tighter and it simply hurt like hell.
NOTE: I am a severe chronic pain patient from autoimmune issues, an untreatable non retractable headache (only med that will help and I'm on is DILAUDID) diagnosed by Jefferson Headache Center in Philadelphia as New Daily Persistent Headache except it isn't a headache as I once knew...no this is constant pressure on the back of my eyes and the worse it gets the more my eyes swell (almost to the point of appearing to be badly swollen from taking a 2 hour beating) and it literally feels as if something (or someone) is inside of my skull driving my eyes out of their sockets from behind. I do get indescribable pain in my temples and forehead too, but the pain from my eyes is unreal. I also have SEVERE lower spinal destruction from L2 through L5/S1 with loads of nerve damage (numerous surgeries) and when flaring my Coccyx feels like someone is literally slamming a 10lb sledge hammer off of the bottom of my tailbone plus my back will become so inflamed I'll wind up incapable of moving anything below my waist when at my worst.

It may just be my body over reacting to any kind of pain but I can't confirm that as I've smashed my hands/fingers thousands of times as well as been punched in the face and none of that stuff even remotely phases me so I don't know....

Hi Italuv, the Cunningham clamp was recommended by the urologist but fitting instructions were from Bard. The importance of moving it along the length of my penis at intervals was stressed but as I am very small and partially retracted there were not many options. I closed it to just press the urethra but still it hurt. The pubic pressure urinal is a much better solution for me and that was professionally fitted by a specialist nurse and although not widely known is a great system if you cannot use a condom catheter and want to be out of adult nappies.

To be honest I tried everything short of surgery to try to beat this but after 3 years and zero improvement (despite numerous medications, not drinking ANY fluids after 6:00 PM) (and I'm ALWAYS thirsty), trying a bed wetting alarm etc I decided "FUCK IT"! Diapers don't EVER cause any pain and are actually quite comfortable. That's what works for me. I don't wear "useless" "DISCREET" diapers after winding up wanting to drop dead when out in public when the useless discreet diaper fails and I wound up with drenched, dripping wet pants and nowhere to hide. Now the most discreet diaper I'll trust is an Abena L4 and with a booster pad if I might not be able to change it within 4-5 hours. IF I'm going to be out all day with the ability to change when needed no booster but when I likely won't be able to change I'll wear a Betterdry with a full sized (Tranquility "Super Plus" contour booster pad) as that's going to last me 8-12 hours without a doubt. No one EVER notices and if they do they don't say a word about it.....granted I always inspect myself in a big mirror before going anywhere to make sure my "underwear" aren't "OBVIOUS" to anyone but outside of that and an adult sized, solid color/short sleeve "onesie" no one ca tell without getting up close & personal to inspect me that my underwear are substantially thicker and much more absorbent than "regular" underwear.

I still wear adult nappies if I know I am going to be seated for a long time i.e. in church, cinema, theatre or a long car journey. I use the Tena Maxi plus a booster pad and waterproof pants as the pubic pressure urinal works best while walking around, shopping etc as gravity is your friend in getting the urine out of the rubber cone and into the leg bag.
 
So there is so many tools or aids out there and like diapers new things do come out. For me it was like a condom cath I didn't have luck with it but it could be few different things. Me personally even though bowel ic is rare during the day even when I used to put in a Foley and use a leg bag I still wore either shapped pad or pull up as just Incase also even with a Foley cath you can have minimum leaks. But because my immune system is crap the risk vs benefit using caths while taking antibiotics didn't work. The main issues I had with condom cath's was Inhad a reaction to the adhesive and if I became retracted it usually fell off. So me personally I really didn't have many options besides potential surgery which isn't guaranteed.

But its good to know what's out there because it's like diapers not everyone needs the mega absorbency ones all the time and some can manage incontinence with pads or liners like I know few people with spinal cord injuries they straight Cath every 3hrs or so but wear a pad just in case. But there are many options out there, I have wondered if people did use other products but do know for ease of use diapers work for many and even though I mainly use diapers myself I have tried shapped pads to allow my skin to breath in past and didnl find like diapers the products made in the EU & China seemed to be better like the makers of Molicare have Moliform pads which basically the same absorbency and similar pattern just doesn't have wings and tapes.

In the UK the nurses at the Continence Advisory Service in each NHS area are absolutely brilliant and it was my nurse who suggested the pubic pressure urinal as I have a very small penis and am partially retracted. The number of aids available can be found on a terrific website called Continence Product Advisor which is a not for profit information service.

I have talked to a few people from the UK and wish that is something the US had!! There are Continence Nurses at a few places but they aren't really that knowledgeable about various aids or devices, the ones I have spoken to mainly knew about cathing and various caths. I know one of the people I spoke with from the UK mentioned something like those Advisors work for the NHS and if you qualify for incontinence aids or products they make the recommendations but the only complaint and I can see both sides is if the Advisor recommends one product that's what they will give you so if they think pullups would work best that's what they will send but if you want a nappy/slip/diaper/brief it's not totally a no but can take time also they may say you only need X amount and if you need more that could be on the person to buy. I am no expert but from what little I have heard it sounds for the most part a good system but as I mentioned in other posts about govt programs none are perfect..


As for being retracted so I asked my Urologist about it because I have it happen few times or I notice when I am changing alot.of the times it is and the I personally have is say I point down when I fasten the diaper if I have a leak usually at the top I believe it's because of that. He explained to me where I am paralyzed and where I am a bigger guy(when I could stand I am 6'5" or 77in -270lbs 122kg) the issue is being overweight my abdominal muscles cause strain or the weight pushes down on my pelvic area which causes that part to retract. He suggested if I lost weight it would help and do know when I first got hurt I weighed less and thinking it wasn't an issue till I did put on weight so he is probably right. I don't know your situation it's not the only cause for it to retract but in my case it was valuable information I figured I would share it was first time I have heard weight could be a cause. I do know my situation maybe different being paralyzed and seated most times & I have a potentially different situation like in have no feeling like belly button down so it's not like I can control things either which besides being ic does really suck more so the blue pills don't work(he had 2 other options but right now one is a definite no and one I will think about)
 
Hi ParaRomeo, you are right about the nurses wishing to give you the items that they feel will meet your needs without overkill. I had previously tried large Tena Comfort pads and net pants but after one appeared from my trouser leg in front of two elderly ladies I swore I would never use them again. The Continence Nurse accepted this and even though I was purchasing my own pull ups told me that all-in-ones would be prescribed free of charge. I was sent samples of Tena Slip Proskin in large and extra large in plus and super and asked what fitted best. The xl super was best but after some leaks we finally agreed on Tena Slip Proskin Maxi and 9 packs of 24 as an initial order. It was while researching other aids available I found the pubic pressure urinal (previously condom catheters just fell off) and discussed this with the Continence Nurse and she arranged for a home visit by a specialist nurse who showed me the options, saw I was partially retracted with a small penis and measured me to enable me to get the best fit. The company, Jade Euro Med then sent everything to the specialist nurse who returned to fit the appliance in my home. I wear it daily if I am going out and have reduced my nappy usage to 5 packs every 3 months being overnight (with plastic pants) and when going out where I will be sitting for long periods along with the plastic pants. At home I can still just manage with a pull up and the Continence Nurse suggested the Beambridge Funnel to stop me spraying urine and directing it into the toilet. The Continence Nurses are fantastic in ensuring you remain comfortable and dry and don't push catheter insertion or anything the patient is uncomfortable with.

That's good to hear like I said wish we had people like that here. But not surprised either because it does seem in General the UK & EU understand IC stuff better like most of the better products offered here are made in UK & EU and even the medical brands like Tena or Attends are far superior few yrs ago I was able to try Tena Maxi Slip and they were amazing compared to the top of the line Tena offered in US and one would think it would be cheaper for a company to make one good product.

Though in my situation being paralyzed and seated and laying down alot probably not an option that would work well for me it's good to know about the products out there because I do occasionally talk to others so it's something someone maybe able to use.
