I don't find that I've ever verbalized it either way but 'in a diaper' sounds more childish and uncontrolled. The diaper has YOU vs. you are wearing IT.

In the few times it has HAD to come up in conversation I've always said 'I have an adult nappy on', that way I 'spose it always sounds like my choice.
