First off I already have urge IC and have been to a doctor i am not trying to cause ic or make it worse. I have a problem where my body tries unsuccessfully to hold it back causing allot of pain. And was wondering if anyone else has experienced this and knows how to get over it? I know it has to do with my brain saying its not ok to let go but it still tries to hold it in even after it has started leaking. It happens way more in public and makes it extremely noticeable if not blatantly obvious.

I don't get pain with my urge IC as a rule, though I did once when I tried to prevent it by pinching my member off (this caused my bladder to spasm). But there are a number of others here who do experience pain with their urge.

Sounds like painful bladder syndrome or interstitial cystitis... And yes the harder you fight the more it hurts it can cause some interesting pain for sure... Also pelvic floor dysfunction can do something quite similar but the pain can radiate to the kidneys and flank and testicles and kind of behind your taint ...it not fun and honestly it very painful and is why I've been in diapers for years that and oab a some nerve issues... I get very little notice if at all before the pain is extreme see a urologist. And btw stress can make it flair so yeah not a good time 4 sure best of luck

There have been a few times where I’ve had some pain trying to hold it in knowing I didn’t have enough protection on. I didn’t want to have an accident. I have been to a urologist and he wants me to have a urolift procedure. My bladder is not emptying out all the way. I have gone to wearing better more absorbent diapers. It has helped me not to have the pain which is very uncomfortable for sure.

You should probably research the pros and cons. I don't think I would use the procedure, myself:
What is the failure rate of Urolift?

1 Crucially, Urolift has been associated with a failure rate of 7% at 2 years and 14% at 4 years, 2 and with the increasing popularity of the Urolift procedure, urologists should be prepared to perform secondary procedures in cases of Urolift failure.

Fortunately I have no pain with my urge incontinence. Of course, if a urge hits and I try to hold too long it will start at some point to hurt (but no spasm) - or, in some cases, the pressure stops after a few seconds, but usually I don't let it go too much to this point - either I have a chance to reach a bathroom or I relax in my diaper

I have urge incontinence but i don't feel that my bladder is full because of nerve damage from Fournier's gangrene debridement surgery
The first I feel is the spasms until my bladder empties enough.
Since i don't enjoy the pain, I relax and enjoy the warm wet feeling.
Since i have enjoyed peeing in my diapers since age 6 i have no reason for not wearing a diaper or getting it fixed.
Every time i get a new doctor, the first thing after reading my medical history they want to check is my Fournier's gangrene scar and i just open my diaper and let them check my foot long scar.
I got Fournier's gangrene as a rare side effect of SGLT2 inhibitor i was taking for diabetes.

I have got NDO, terminal detrusor over activity and suspected interstitial cystitis.

Until i got Botox and went to a special diet the pain was insane. It started at about 100ml in the bladder and raised continuously up to 250ml, at this point the bladder would spasm and the pain was like stabbing a knife down there, it took down me to my knees sometimes.

Now it is much, much better, some flare ups, but it gets only painful when the bladder reaches 350ml with only minor pain before that, but still it is no normal urge i think.

When the problems begun I had heavy pain but not only when the bladder cramps. Meanwhile it‘s somehow better. It is more like when a muscle cramps but then only for maybe 30 or 40 seconds - and then it‘s over. For 4 or 5 times a day this bearable. In fact this is much better then before where it hurts somtimes permanetly over the complete day and also at the night. I think at the end it hardly depends on the root cause. For me the bladder nerves are meanwhile pretty numb so I don‘t feel that much anymore. How ever - for those with OAB it often helps to „overright“ the actual urge by a different sensation. For me one trick that worked sometimes was to massage my achilles tendon.
