The padding in diapers tends to deform over time. The rate varies by diaper and conditions. Some diapers are prone to losing padding cohesion, particularly when wet, which is often referred to as "clumping" due to the disconnected clumps of padding in the diaper. A good mix of fiber and Super Absorbent Polymer (SAP) minimizes this but it's pretty inevitable given time and wear.
 
try doing it with your neck in a brace and a broken dominant arm...
 
For me, diapering myself is a laborious process since I have Cerebral Palsy. In bed, I diaper myself lying down, and in the bathroom a mixed seated/standing hybrid method to diaper myself. During diaper changes though, I do try not to let the mess fall out and plop onto my AFO leg braces and the floor.
