That's just the thing about fetishes, not everyone has them. But people do have diaper fetishes. It may not be your thing (or mine), but for some it is. And the key factor for anyone with a fetish is that yes they do need to at the very least think about that fetish in order to reach a climax. That's just how fetishes work.
And yes, mastubating a lot will make it more difficult to get aroused in general.
As Slomo said...
paraphilia:
unconditional
requirement of presence of unusual sexual objects, sexual practices or circumstances, as beeing the (
unwillingly)
requirement of sexual arousal.
fetishism:
a paraphilia, using (non-living) objects, with whom the person had
an intimate coincidence in the past, to achieve sexual arousal
causes:
one theoretical model of developing paraphilia:
paraphilia are a consequence of a similar coincidence in
childhood as beeing the
first sexual experience
another theoretical model of developing paraphilia:
conditioning, causal to sexual phantasies, or memories based on
early (
childhood) sexual experience

For example:
Imagine a toddler (strong sexual arousal already possible) rocking a seasaw (etc.) at the playground, in his wet diapers becoming intensively pleasured
by movement, friction and pressure - wanting to do it again and again, etc...
As you can see there are many possible
childhood coincidences beeing capable to cause diaper fetishism...
- getting your diapers changed
- baby swing
- running/playing in messy diapers and tight clothes
- driving in wet diapers, while beeing locked in a 5-point-harness
