To answer the OP's question of course no it's not illegal to change someones nappy in a toilet, although you might get some questions. from people, on a slightly different note I once got accused of being a drug user many years ago by a security guard in a shopping centre whilst I was using a disabled toilet, he didn't know where to put himself when I left when he saw I was a wheelchair user so had a genuine need for the toilet
Anyone can use a disabled toilet in the UK, in fact to save space some shops like coffee shops just install a disabled toilet as they are normally unisex and anyone can use it, these ones don't normally have a radar lock on.
It's pretty easy to get hold of a RADAR key, I can get one from my local council for £3, you don't need a Dr's note. You can also buy them from
Disability Rights UK, they are the successor to RADAR as a charity and are the guardians of the Radar Key scheme, they also send them internationally.
Also some stores have been putting these labels on their disabled toilet doors to demonstrate that disabilities can be hidden:

Finally whilst we're on the subjects of toilets, I did find it weird when I was on holiday in the USA that the Disabled toilet was in with the rest, I say that because coming from the UK where almost all of our disabled toilets are separate, I also found it awkward having to wait for the wider stall to become free because I'm a wheelchair user( I've got Cerebral Palsy), although I did just about manage to get a scooter within a stall in a Las Vegas casino somehow.
