Two things from English experience. a lot of "undiapered" people are fearful of wetting or pooping in bed - especially as nursing staff cannot always get to them quickly enough - If you are diapered you don't have that worry, or even the indignity of being told "just go if you can't wait and we'll clean you later". Staff are incredibly busy and would probably rather have a known quantity like attending to diaper needs, than an unknown one of upset and embarrassed patients demanding time they don't have.
Hospitals are not like other places - at a out-patents waiting room I once saw a guy poop on the floor. No one turned an eyelid and both he and the floor were attended to with dispassionate and effective action designed to protect his feelings.
