My perfect date:

Just me and him/her. We go out to a nice italian restaurant, and share a lovely bottle of wine. Then we head out to a plaza at sunset and just sit around the fountain and bs. The night ends with us sitting at home by candlelight or quiet flame, just enjoy our closeness before eventually I pass on their shoulder (I suck at staying awake :P).

I love food, a lot. So the ideal date would definitely involve going out to dinner/getting drinks.

Other then that, stuff like bowling/mini golf/video games can be fun and aren't really too awkward.
